This is a fully remote position, open to applicants in Colorado.
• Operate independently while collaborating with all healthcare team members.
• Organize and facilitate high-quality, cost-effective care for patients with ESRD and CKD.
• Reduce fragmentation within the healthcare delivery system.
• Coordinate care among patients, families, physicians, providers, care teams, the community, and the Regional Operations Manager.
• Provide case management services via telephonic, field-based, or a combination of both methods.
• Support integrated care management for kidney disease patients across the United States.
• Assist in preventing complications, decreasing avoidable hospitalizations, and enhancing overall health.
• An A.D.N degree from an accredited nursing school is required; a B.S.N is preferred.
• A current Registered Nurse (RN) license in the state of practice is mandatory.
• Must be open to adjusting work hours to facilitate patient outreach as necessary.
• A minimum of 1 year of experience as an IKC RN is required.
• Must be able to work a Monday-Friday schedule.
• Capability to work independently is essential.
• Continuing education credits must be maintained as required by the state of practice.
• At least three (3) years of clinical nursing experience is required.
• A minimum of one (1) year of renal nursing or case management experience is preferred.
• Proven knowledge and understanding of data management related to clinical, financial, and patient satisfaction outcomes.
• Demonstrated effectiveness in a change agent role.
• Knowledge of and experience with CQI techniques is essential.
• Current CPR certification is required.
• Ability to adapt to new electronic processes and meet increasing productivity expectations.
• Basic computer skills and proficiency in MS Word and Outlook are necessary.
• Functional proficiency with DaVita-specific clinical software programs, including Capella, is required within 90 days of employment.
• A home office with internet connectivity of at least 1MB upload and download speed is required.
• A Compact License is preferred.
