This is a fully remote position, open to applicants in United States.
• Deliver nursing care and manage care coordination for a specific patient demographic identified through risk stratification reports.
• Present findings, trends, and recommendations related to population health to client leadership and matrix partners.
• Engage in client meetings to evaluate outcomes, intervention efficacy, and emerging health priorities.
• Collaborate with client stakeholders to prioritize initiatives and strategies for population health engagement.
• Conduct comprehensive biometric health risk assessments and provide education.
• Adhere to established nursing protocols and procedures.
• Monitor medication adherence and offer patient education.
• Inform patients about chronic conditions, abnormal biometric values, health necessities, and lifestyle modifications.
• Utilize motivational interviewing techniques to encourage sustainable dietary, exercise, and stress management changes.
• Assess customer health education needs and identify psychosocial barriers to care.
• Develop health improvement plans and personalized, evidence-based health goals in collaboration with patients.
• Track and oversee compliance with screening protocols.
• Facilitate preventive health screenings, such as cancer screenings.
• Lead and support promotional activities related to health and wellness, group coaching, wellness challenges, seminars, and wellness campaigns at client sites.
• Coordinate communication with external healthcare providers, including primary care physicians and specialists.
• Collaborate with community providers and resources to meet patient needs and address care gaps.
• Reinforce adherence to provider care plans.
• Follow up with patients regarding recommendations for after-hours care.
• Refer patients to clinical programs and services as necessary.
• Assist patients in navigating benefits and accessing essential resources.
• Advocate for timely and appropriate healthcare services.
• Support health-related events at the site, including open enrollment, wellness committees, flu shot clinics, and health fairs.
• Utilize reports and dashboards to identify high-risk patients and proactively engage them in care management.
• Prioritize patient outreach and interventions based on risk stratification.
• Ensure adherence to regulatory requirements.
• Complete necessary electronic documentation in accordance with policies and procedures.
• Unrestricted compact state RN licensure is required.
• A minimum of three years of clinical practice experience is necessary.
• A bachelor's degree in a health-related field is required.
• Current BLS/CPR/AED certification is mandatory.
• Proficient computer skills, including Word and Excel, are essential.
• Strong interpersonal skills are required.
• Ability to work collaboratively within a team environment is necessary.
• Flexibility and eagerness to learn new information and quickly develop new skills are important.
• A high energy level and a passion for health improvement are essential.
• Serve as a positive role model by demonstrating healthy behaviors.
• Self-motivated with the ability to thrive in an independent role.
• Experience in population health management, care management, clinical program management, and/or health analytics is preferred.
• Care management or case management certification is preferred.
• Familiarity with healthcare analytics and reporting tools is preferred.
• Knowledge of employer-sponsored health plans and wellness strategies is preferred.
• Experience with Epic, population health platforms, or care coordination systems is preferred.
• A reliable broadband or fiber optic internet connection with a minimum of 10Mbps download and 5Mbps upload speed is required for remote work.
