This is a fully remote position, open to applicants in United States.

📋 Description

• Deliver nursing care and care coordination to a specified population identified through risk stratification reports.

• Execute comprehensive health risk assessments for patients and provide education on health and benefits navigation.

• Present findings, trends, and recommendations regarding population health to client leadership and matrix partners.

• Engage in client meetings to evaluate outcomes, intervention effectiveness, and emerging health priorities.

• Collaborate with client stakeholders to prioritize initiatives and strategies for population health engagement.

• Conduct biometric health risk assessments and deliver educational content.

• Adhere to established nursing protocols and procedures.

• Supervise medication adherence and offer patient education.

• Inform patients about chronic conditions, abnormal biometric readings, health requirements, and lifestyle modifications.

• Utilize motivational interviewing techniques to encourage sustainable changes in diet, exercise, and stress management.

• Assess health education needs through targeted evaluations.

• Identify and assist in overcoming psychosocial barriers to care.

• Develop health improvement plans and personalized evidence-based objectives with patients.

• Monitor and track compliance with screening.

• Facilitate preventive health screenings, including cancer screenings.

• Lead and support wellness initiatives, group coaching, wellness challenges, seminars, and campaigns on client sites.

• Coordinate communication with external providers, including primary care and specialists.

• Collaborate with community providers and resources to meet patient needs and close care gaps.

• Reinforce provider care plans and ensure adherence.

• Follow up with patients regarding after-hours care recommendations.

• Refer patients to relevant clinical programs and services.

• Assist patients in navigating benefits and accessing resources.

• Advocate for timely and appropriate patient care.

• Support health-related site events such as open enrollment, wellness committees, flu shot events, and health fairs.

• Utilize reports and dashboards to identify and proactively engage high-risk patients.

• Prioritize outreach and interventions based on risk stratification.

• Ensure compliance with regulations and maintain appropriate electronic documentation.


⛳️ Requirements

• Must possess current unrestricted RN licensure in a compact state.

• A minimum of three years of clinical practice experience is required.

• Bachelor's degree in a health-related discipline is necessary.

• Current BLS/CPR/AED certification is required.

• Proficient computer skills, particularly in Word and Excel.

• Exceptional interpersonal skills are essential.

• Ability to thrive in a team-oriented environment.

• Flexibility and eagerness to quickly learn new information and develop new skills.

• High energy level and a passion for health improvement.

• Serve as a positive role model by demonstrating healthy behaviors.

• Self-motivated and capable of succeeding in an independent role.

• Preferred experience in population health management, care management, clinical program management, and/or health analytics.

• Care management or case management certification is preferred.

• Experience with healthcare analytics and reporting tools is advantageous.

• Knowledge of employer-sponsored health plans and wellness strategies is preferred.

• Familiarity with Epic, population health platforms, or care coordination systems is advantageous.

• Must have cable broadband or fiber optic internet service with minimum speeds of 10Mbps download/5Mbps upload for remote work.
