Registered Nurse – Home Health

This is a fully remote position, open to applicants in Arizona.

📋 Description

• Deliver personalized home health care to elderly patients in their residences or care facilities.

• Collaborate closely with the Director of Nursing to create tailored care plans.

• Address each patient's specific goals and requirements.

• Conduct visits lasting 45–60 minutes during 9-week home health care episodes.

• Offer additional compassionate hospice services as needed.

• Travel across the designated service area and assigned coverage territory.

• Participate in one weekend on-call rotation each month.

• Provide high-quality, patient-focused care to communities in rural Arizona.

⛳️ Requirements

• Current Arizona RN License.

• Valid Driver's License.

• Up-to-date Auto Insurance.

• Arizona Level One Fingerprint Clearance Card.

• Valid CPR or BLS Certification.

• Recent TB Test (skin test within the last year or chest x-ray within the last 5 years).

• Willingness to travel throughout the service area.

• Capability to provide coverage in the Peoria Buckeye area.

• Availability for one weekend on-call rotation per month.

• IV experience is preferred.

• Background in providing home health or hospice nursing care to elderly patients.
