This is a fully remote position, open to applicants in Wisconsin.
• Evaluate the health and safety requirements of members to create comprehensive Member Care Plans.
• Perform regular health assessments.
• Collaborate with acute and primary care providers.
• Inform members about disease processes, risks, nutrition, exercise, and lifestyle changes.
• Maintain documentation in member files within the mandated DHS contract deadlines.
• Engage in monthly member phone communications and travel for face-to-face member visits.
• Work remotely and travel approximately 40% of the time to visit members.
• Conduct record reviews for reassigned members and pinpoint necessary remediation.
• Finalize documentation for evident remediation updates.
• Notify newly assigned RNs and Care Coach partners about missed or required follow-up.
• Assign tasks in the D365 system when necessary.
• Offer assistance for record review and remediation processes.
• Support the team through changes in procedures.
• Take on additional complex member assignments.
• Provide further assistance with new member assignments.
• Alert Managers to significant risks or concerns discovered during record reviews.
• Associate degree in nursing.
• Registered Nurse license in the state of Wisconsin (in good standing).
• Preferred: 1 year of Care Management experience.
• Preferred: Reside within 45 minutes or less from the designated coverage area.
• Previous Care Management experience is preferred.
• Valid state driver's license.
• Proof of personal vehicle liability insurance with minimum limits of 100/300/100.
• Required to undergo TB screening if selected.
• Minimum home internet speed of 25 Mbps download and 10 Mbps upload.
• Dedicated workspace free from ongoing interruptions to safeguard member PHI / HIPAA information.
