Registered Nurse – Care Manager

This is a fully remote position, open to applicants in Wisconsin.

📋 Description

• Evaluate the health and safety requirements of members to formulate detailed Member Care Plans

• Perform regular health evaluations

• Collaborate with both acute and primary care providers

• Provide education to members regarding disease processes, associated risks, nutrition, exercise, and lifestyle changes

• Maintain documentation in member files within the mandated DHS contract schedules

• Engage in monthly phone outreach to members

• Travel to members' locations for face-to-face visits

• Assist members enrolled in the Wisconsin Family Care Program through care coordination, support, and education

• Work remotely from home while traveling approximately 40% to meet members

⛳️ Requirements

• Associate's degree in nursing

• Registered Nurse license in the state of Wisconsin, in good standing

• Preferred: 1 year of experience in Care Management

• Preferred: Residence within 45 minutes or less of the designated coverage area

• Preferred: Prior experience in Care Management

• Valid driver's license issued by the state

• Proof of personal vehicle liability insurance with minimum limits of 100/300/100

• Capability to meet patient-facing TB screening criteria

• Self-provided internet connection with at least 25 Mbps download and 10 Mbps upload speeds

• Dedicated workspace that ensures no interruptions to safeguard member PHI/HIPAA information

• Availability to work Monday through Friday, from 8:00 am to 4:30 pm CST

• Willingness to travel up to 40%
