This is a fully remote position, open to applicants in United States.
• Collaborate with patients by evaluating, facilitating, planning, and advocating for their unique health requirements.
• Monitor referrals and maintain accurate documentation to enhance team awareness and ensure patient safety.
• Conduct precise patient registration, which includes gathering demographic data and current insurance details.
• Compile clinical histories and referral needs, providing relevant clinical information to specialists.
• Communicate with review organizations and insurance providers to fulfill prior-approval criteria.
• Present medical details such as patient history, diagnosis, and prognosis.
• Supply medical information to financial services to optimize reimbursement for hospitals and physicians.
• Discuss referral specifics and expectations with patients.
• Assist patients in overcoming healthcare-system, financial, and social obstacles, including access to interpreters, transportation, and prescription support.
• Act as a system navigator and primary contact for patients and their families.
• Advocate with insurers for prompt approval of necessary patient supplies and services.
• Identify and leverage cultural and community resources.
• Build and maintain relationships with service providers.
• Ensure timely processing of referrals and remind patients of their appointments through mail or phone.
• Update primary care charts with specialist consultations, hospital admissions, emergency room visits, and pertinent community organizations.
• Adhere to the Code of Conduct and HIPAA regulations.
• High school diploma, sometimes combined with certification as a medical assistant.
• Strong focus on customer service.
• Excellent verbal and written communication abilities.
• Orientation towards teamwork.
• Well-organized and capable of managing competing priorities.
• Good judgment skills.
• Resourceful in problem-solving situations.
• Ability to take responsibility for delegated tasks and follow through on them.
• Bilingual skills are preferred (English/Spanish).
• Must have authorization to work in the U.S.; the employer participates in E-Verify.
