
Re-Entry Care Coordinator

This is a fully remote position, open to applicants in North Carolina.
• Oversee and coordinate care transition activities for individuals listed on the High Priority Re-entry list.
• Assist in ensuring successful discharges and service modifications.
• Create and execute transition plans tailored to individual needs and requirements.
• Work in partnership with providers and community partners to synchronize services during transition phases.
• Supervise transitions and take action to address any delays or obstacles.
• Facilitate service connections to maintain continuity after transitions.
• Provide support for individuals with justice involvement who are transitioning between care settings and preparing for release from Department of Corrections or county detention facilities.
• Offer care coordination and transition planning services.
• Identify and address barriers to ensure successful and stable transitions.
• Must fulfill Qualified Professional (QP) status according to NC requirements.
• A minimum of one (1) year of experience working with the forensic population (individuals involved in the criminal justice system).
• One acceptable pathway: fully licensed by the NC governing board overseeing Human Services professions; OR fully licensed RN authorized to practice in North Carolina by the NC Board of Nursing with four (4) years of full-time experience in the MH/DD/SA sector; OR a master's degree in a human service field combined with one year of full-time MH/DD/SA experience or qualifying supervised substance-abuse counseling experience; OR a bachelor's degree in a human service field with two years of full-time MH/DD/SA experience or qualifying supervised substance-abuse counseling experience; OR a bachelor's degree in another field with four years of full-time MH/DD/SA experience or qualifying supervised substance-abuse counseling experience.
• Must possess a valid driver’s license.
• Must reside in North Carolina and be located within 75 miles of New Hanover County.
• Must be willing to travel within the designated area as necessary.
• Preferred: Degree in Human Services or Criminal Justice.
• Education, training, and work experience must demonstrate the ability to engage with individuals from diverse backgrounds.
• All candidates are required to pass a drug test as a prerequisite for employment.
