
Re-Entry Care Coordinator

This is a fully remote position, open to applicants in North Carolina.
• Oversee and coordinate care transition activities for individuals on the High Priority Re-entry list.
• Create and execute transition plans tailored to individual needs and specific transition requirements.
• Collaborate with service providers and community partners to synchronize services during transition phases.
• Monitor the transition process and take action to address any delays or obstacles.
• Facilitate connections to services and ensure continuity after transitions.
• Assist justice-involved individuals transitioning from the Department of Corrections or county detention facilities.
• Must fulfill the Qualified Professional (QP) criteria according to NC regulations.
• A minimum of one (1) year of experience working with the forensic population.
• Must possess a full license from the NC governing board that regulates Human Services professions; OR be a fully licensed RN authorized to practice in North Carolina by the NC Board of Nursing with four (4) years of full-time experience in the MH/DD/SA population; OR hold a master’s degree in a human service field with one year of full-time experience in MH/DD/SA; OR have a bachelor’s degree in a human service field with two years of full-time experience in MH/DD/SA; OR a bachelor’s degree in a different field with four years of full-time experience in MH/DD/SA.
• Must hold a valid driver's license.
• Must reside in North Carolina and be located within 75 miles of Onslow County.
• Must be willing to travel within the designated catchment area as needed.
• Preferred: Degree in Human Services or Criminal Justice.
• Education, training, and work experience that demonstrates the ability to engage with individuals from diverse backgrounds.
• All candidates must successfully pass a drug test as a prerequisite for employment.
