
Re-Entry Care Coordinator

This is a fully remote position, open to applicants in North Carolina.
• Oversee and coordinate care transition activities for individuals on the High Priority Re-entry list.
• Assist in facilitating successful discharges and changes in service.
• Create and execute transition plans tailored to individual needs and requirements.
• Work in partnership with providers and community organizations to synchronize services during transition phases.
• Track transitions and take action to address any delays or obstacles.
• Enable service connections and ensure continuity of care following transitions.
• Provide care coordination and transition planning for individuals involved with the justice system who are preparing for release from the Department of Corrections or county detention centers.
• Tackle barriers to ensure successful and stable transitions.
• Must fulfill the Qualified Professional (QP) criteria as outlined by North Carolina regulations.
• Minimum of one year of experience working with the forensic population.
• One of the following qualifying pathways: fully licensed by the NC governing board overseeing Human Services professions; or a fully licensed RN eligible to practice in North Carolina with four years of full-time experience in MH/DD/SA populations; or a master's degree in a human service discipline with one year of full-time MH/DD/SA experience; or a bachelor's degree in a human service field with two years of full-time MH/DD/SA experience; or a bachelor's degree in a different field with four years of full-time MH/DD/SA experience.
• A valid driver's license is required.
• Candidate must reside within North Carolina and specifically in Trillium's South Central region, which includes Carteret, Craven, Duplin, Jones, Lenoir, Onslow, Pender, Sampson, or Wayne counties.
• Ability to travel within the designated catchment area as necessary.
• Preferred qualifications include a degree in Human Services or Criminal Justice.
• Must successfully pass a drug screening as a condition of employment.
