
Rapid Response Care Advocate – 24/7 Urgent Patient Support

This is a fully remote position, open to applicants in United States.
• Manage high-urgency patient cases that have already been triaged.
• Review each case to formulate an execution plan with prioritized, time-sensitive actions.
• Implement the plan utilizing provided resource directories and independently seek additional resources as necessary.
• Keep the healthcare operations team informed about progress, challenges, and any support required.
• Contact patients to update them on progress, clarify details, and offer reassurance.
• Accurately document every interaction and support activity within the platform.
• Adhere to confidentiality and comply with HIPAA, privacy, and platform standards.
• Remain accountable for urgent cases until resolution, typically within 2–3 days.
• Collaborate with a 24/7 team covering nights, weekends, and holidays.
• Possess an active LPN/LVN license; or Community Health Worker (CHW) certification; or have equivalent experience in community health or patient navigation.
• A minimum of 2 years of experience in patient or community-facing support roles.
• Familiarity with the U.S. healthcare system and insurance practices.
• Capability to work independently while managing multiple urgent cases at once.
• Excellent documentation skills.
• Access to a reliable computer, internet connection, and a quiet environment for patient calls.
• Commitment to at least 20 hours of work per week.
• Availability for shifts that include evenings, weekends, and holidays, along with an overnight on-call rotation.
• Ability to maintain confidentiality and adhere to HIPAA, privacy, and relevant platform standards.
• Strong verbal and written communication skills.
• Preferred: experience assisting older adults or patients with complex or serious health issues.
• Preferred: background in urgent or high-tempo environments.
• Preferred: knowledge of Medicaid and other public health programs.
• Preferred: bilingual proficiency in Spanish or another language commonly spoken by patients served.
• Option for remote work.
