PTA, Care Navigator

This is a fully remote position, open to applicants in New York.

📋 Description

• Deliver remote monitoring services that assist patients in adhering to home-exercise therapy programs.

• Guide and support users in therapeutic exercises, neuromuscular re-education, and functional training utilizing the technology platform.

• Provide continuous feedback to the evaluating clinician regarding patient progress outside of clinic settings.

• Keep accurate records and notes while performing remote monitoring duties.

• Contribute to the enhancement of platform processes and functionalities.

• Efficiently organize and manage a caseload.

⛳️ Requirements

• Graduated from an accredited physical therapy program.

• Hold an active PTA license in New York.

• Proficient with technology; comfortable navigating EHR, messaging, and scheduling platforms.

• Exceptional communication, interpersonal, and presentation abilities.

• Capable of gathering and presenting relevant data to inform decision-making.

• Strong skills in relationship-building and effective listening.

• Ability to multitask and manage a diverse range of users.

• Capable of working independently with minimal supervision.

• Flexible and adaptable to work cross-functionally in a dynamic environment.

• Empathetic, approachable, and focused on patient needs.

• Genuine enthusiasm for user experience and engagement.

• Ability to drive change and bring positivity to the workplace.

• Independent, proactive work ethic with quick and adaptable thinking.

• Open-minded and curious approach to challenges.

• Passionate about health and wellness.

• 2+ years of experience in musculoskeletal health is advantageous.

• Background in behavioral modification is a plus.

• Health Coach Certification is a bonus.
