PTA, Care Navigator

This is a fully remote position, open to applicants in New York, +1 more state.

📋 Description

• Deliver remote monitoring services for individuals participating in home-exercise therapy programs.

• Guide and support users in therapeutic exercises, neuromuscular re-education, and functional training through our technology platform.

• Provide continuous feedback to evaluating clinicians regarding patient progress outside of the clinical setting.

• Keep necessary records and notes while conducting remote monitoring activities.

• Aid in enhancing platform processes and functionalities.

• Efficiently organize and manage a caseload.

⛳️ Requirements

• A minimum of 2 years of experience in musculoskeletal health is preferred.

• Must have graduated from an accredited physical therapy program.

• Hold an active PTA license in New York.

• Proficient with technology; comfortable using EHR, messaging, and scheduling systems.

• Experience in behavioral modification is advantageous.

• Health Coach Certification is a bonus.

• Exceptional communication, interpersonal, and presentation skills.

• Capable of gathering and presenting significant data to inform decision-making.

• Strong skills in relationship-building and active listening.

• Proven patient-centric approach.

• Ability to manage multiple tasks and engage with various types of users.

• Capacity to work independently with minimal supervision.

• Adaptability to work cross-functionally in a dynamic environment.

• Independent and proactive work ethic; quick and flexible thinking; open and inquisitive mindset.
