PTA, Care Navigator

This is a fully remote position, open to applicants in New York.

📋 Description

• Deliver remote monitoring services for patients engaged in home-exercise therapy programs.

• Guide and assist users in therapeutic exercises, neuromuscular re-education, and functional training via the technology platform.

• Provide continuous feedback to evaluating clinicians regarding patient progress outside of the clinic.

• Keep necessary records and notes while conducting remote monitoring.

• Contribute to enhancing platform processes and functionality.

• Efficiently organize and manage a caseload.

• Recognize patient needs and challenges, assisting in the implementation of interventions to promote behavior change.

• Build trusting relationships with clients and patients.

⛳️ Requirements

• Graduated from a recognized physical therapy program.

• Current PTA license in New York.

• Over 2 years of experience in musculoskeletal health is advantageous.

• Proficient with technology; familiar with EHR, messaging, and scheduling platforms.

• Background in behavioral modification.

• Health Coach Certification is beneficial.

• Exceptional communication, interpersonal, and presentation abilities.

• Capability to gather and present meaningful data to inform decisions.

• Strong skills in relationship-building and active listening.

• Proven patient-centered approach.

• Ability to multitask and manage various user types.

• Capacity to work independently with minimal supervision.

• Flexibility and adaptability to work collaboratively in a dynamic environment.

• Independent and proactive work ethic; quick, adaptable thinking; open-minded and curious attitude.
