This is a fully remote position, open to applicants in Tennessee.
• Act as the main point of contact for designated practices.
• Foster trust and alignment around value-driven objectives.
• Analyze and interpret clinical performance data with healthcare providers.
• Identify trends, opportunities, and exemplary practices.
• Assist practices in executing improvement plans focused on outcomes, utilization, and patient engagement.
• Recognize and escalate performance obstacles while collaborating internally on solutions.
• Educate and mentor providers and staff on Chamber’s platform.
• Ensure effective usage of tools for care coordination, documentation, and reporting.
• Monitor user engagement and adoption metrics.
• Tackle underutilization through targeted strategies.
• Convert customer feedback into product enhancements in collaboration with Product and Operations teams.
• Oversee onboarding and continuous success planning for practices.
• Develop scalable workflows and success playbooks.
• Track engagement, gap closure, and quality performance metrics.
• Provide regular progress reports to stakeholders.
• Set up recurring success reviews and lead joint metric assessments.
• Conduct training sessions at the practice level.
• Create feedback loops between practices and internal teams.
• 5–8+ years of experience in customer success, practice management, or provider engagement, preferably in value-based care, managed care, or population health.
• Comprehensive understanding of clinical workflows, quality metrics, and payer-provider collaboration.
• Outstanding relationship management and communication abilities, with a consultative and empathetic approach.
• Capability to interpret and communicate healthcare data to non-technical audiences.
• Enthusiasm for technology, particularly in implementing new tech and AI solutions.
• Detail-oriented operational mindset that balances proactive planning with responsive problem-solving.
• Commitment to enhancing outcomes and experiences in cardiovascular care.
• Eligibility to work in the US without the need for visa sponsorship.
• Willingness to travel occasionally to practice locations or company offices.
• Flexible remote work arrangement.
• Occasional travel to practice sites or Chamber offices may be necessary.
