
Provider Performance Manager

This is a fully remote position, open to applicants in Illinois.
β’ Work collaboratively with healthcare providers to enable them to excel in value-based contracts.
β’ Partner with provider staff to create and implement enhancements in operational and clinical processes.
β’ Identify, develop, and establish essential performance metrics related to providers' value-based agreements.
β’ Recognize improvement opportunities based on key performance indicators and formulate action plans.
β’ Create and deliver training and support to provider staff concerning performance metrics and operational and clinical enhancements.
β’ Foster collaboration among provider groups to exchange best practices and opportunities for performance improvement.
β’ Address provider feedback and concerns by resolving issues related to systems, processes, or resources.
β’ Undertake additional duties as assigned.
β’ A Bachelor's Degree in Nursing, Medicine, or Health Sciences is required.
β’ A minimum of 3 years of experience in behavioral health, value-based contracting, or process and performance improvement is required.
β’ A Master's Degree in Nursing, a Physician Assistant degree, or a graduate degree in Pharmacy, Social Work, Counseling, or Clinical Psychology is preferred.
β’ Willingness to travel (25%) throughout the Chicago metropolitan area, particularly in Cook County and surrounding collar counties, mainly DuPage, Lake, and Will counties.
