This is a fully remote position, open to applicants in Illinois.

📋 Description

• Work collaboratively with healthcare providers to enable them to excel in value-based contracts.

• Partner with provider staff to create and implement enhancements in operational and clinical processes.

• Identify, develop, and establish essential performance metrics related to providers' value-based agreements.

• Recognize improvement opportunities based on key performance indicators and formulate action plans.

• Create and deliver training and support to provider staff concerning performance metrics and operational and clinical enhancements.

• Foster collaboration among provider groups to exchange best practices and opportunities for performance improvement.

• Address provider feedback and concerns by resolving issues related to systems, processes, or resources.

• Undertake additional duties as assigned.

⛳️ Requirements

• A Bachelor's Degree in Nursing, Medicine, or Health Sciences is required.

• A minimum of 3 years of experience in behavioral health, value-based contracting, or process and performance improvement is required.

• A Master's Degree in Nursing, a Physician Assistant degree, or a graduate degree in Pharmacy, Social Work, Counseling, or Clinical Psychology is preferred.

• Willingness to travel (25%) throughout the Chicago metropolitan area, particularly in Cook County and surrounding collar counties, mainly DuPage, Lake, and Will counties.
