This is a fully remote position, open to applicants in United States.
• Lead a team dedicated to providing comprehensive, patient-centered care for individuals with complex and chronic conditions, such as kidney disease, diabetes, heart failure, and other comorbidities.
• Offer collaborative and supportive leadership while managing the daily operations of the care management team.
• Ensure the consistent application of proactive care strategies aimed at enhancing patient outcomes.
• Utilize clinical and operational expertise to oversee the management of high-risk populations.
• Guarantee compliance with established care pathways.
• Aim to decrease complications, prevent avoidable hospital admissions, and reduce overall healthcare utilization.
• Drive the performance of the CCPM program in relation to key performance indicators (KPIs) and quality metrics.
• Foster team development through coaching, training, and performance management.
• Supervise the daily operations of CCPM care management teams, ensuring effective workflows, standardized processes, and attainment of program goals.
• Ensure the provision of high-quality longitudinal chronic/complex care management, adhering to clinical pathways, protocols, and documentation standards.
• Assess and enhance the performance of the CCPM program and team members.
• Formulate and execute strategies to boost program performance, chronic disease management, complication prevention, reduced readmissions, and satisfaction among patients and provider partners.
• Assist in the orientation and onboarding of newly hired team members.
• Conduct regular audits of calls and documentation.
• Prepare performance reports, dashboards, and meeting materials for both internal and external stakeholders.
• Cultivate relationships with key stakeholders and promote cross-program collaborations.
• Develop communication strategies between Evergreen and market stakeholders.
• Nurture a culture characterized by high performance, transparency, trust, and collaboration.
• Carry out additional responsibilities as assigned.
• Bachelor’s degree in healthcare or nursing (BSN required).
• Active state nursing license in good standing.
• A minimum of 3 years of experience in longitudinal clinical nursing care management is required.
• At least 2 years of experience in a managerial or leadership role within clinical nursing.
• Experience in longitudinal care management.
• Strong understanding of value-based healthcare operations related to disease management and population health programs.
• Knowledge of Business Continuity and the ability to integrate resilience into business processes and systems.
• Capability to develop talent, delegate effectively, innovate, and iterate with your team, leveraging their strengths.
• Exceptional skills in relationship-building, communication, and change management, with the ability to influence effectively and establish trust with both internal and external partners.
• Strong leadership presence that inspires confidence in peers, external partners, board members, and team members.
• Execution-focused and hands-on, with the ability to balance strategic priorities alongside detailed day-to-day operations.
• Ability to motivate staff and influence outcomes while promoting team building.
• Proven ability to work effectively both independently and collaboratively in team settings.

• Strong organizational skills, operational rigor, resilience, and dedication to the business.
• Proficient in the MS Office Suite, including Outlook and Teams.
• Ability to work effectively in a remote setting.
• Home internet must support a minimum download speed of 25 Mbps and upload speed of 10 Mbps.
• Employees must work from a dedicated space that minimizes ongoing interruptions to protect member PHI / HIPAA information.
• Master’s degree in nursing, healthcare administration, or a related field is preferred.
• Experience in quality improvement utilizing data to monitor and enhance program performance is preferred.
• Previous experience in a managed care environment, particularly in an administrative or management capacity, is preferred.
• Experience leading the clinical function of a Value-Based Care organization is preferred.
• Availability to work from 8:30 am to 5:00 pm in the Central or Eastern time zone.
