Primary Care Social Work Case Manager

This is a fully remote position, open to applicants in United States.

📋 Description

• Conduct telephonic and video consultations with patients and their caregivers.

• Build trust and foster strong relationships with patients.

• Collaborate with clinicians, nurses, and care coordinators to deliver cohesive, patient-centered care.

• Implement social work case management interventions.

• Execute psychosocial screenings and develop personalized care plans.

• Offer emotional support and assistance to caregivers.

• Guide family and caregiver support initiatives.

• Address social determinants of health and factors influencing health utilization.

• Provide navigation for behavioral health services.

• Support both long-term care planning and advance care planning initiatives.

• Manage multiple competing priorities from patients effectively.

• Assist family members and caregivers in understanding patient needs and how to support them.

• Facilitate and engage in joint visits.

• Participate in team huddles, panel reviews, interdisciplinary team rounds, and contribute to program and role development.

• Build relationships and coordinate with key local market and internal stakeholders.

• Record all clinical interactions in the electronic health record.

• Master’s degree in social work and an active license.

• At least 2 years of experience in the medical field.

• Proficient with technology and able to thrive in a fast-paced environment.

• Self-motivated with the ability to excel in a remote work setting.

• Excellent interpersonal and communication abilities.

• Comfortable engaging with patients via video and phone.

• Bilingual in English and Creole, Spanish, or Vietnamese is a plus.

• Experience in health insurance (Medicare Advantage experience is a plus).

• Familiarity with geriatrics is advantageous.
