Pregnancy Case Manager III

This is a fully remote position, open to applicants in Arizona, +16 more states.

📋 Description

• Deliver member-focused, evidence-informed pregnancy case management across various products

• Perform thorough assessments

• Create, execute, and oversee personalized care plans

• Coordinate services throughout the healthcare spectrum

• Provide education and care coordination

• Identify care gaps and advocate for maternal members

• Assist in minimizing pregnancy- and birth-related complications as well as healthcare expenses

• Communicate with physicians and healthcare providers

• Negotiate and secure resources

• Uphold the organization’s mission, vision, goals, and strategies

• Make decisions under pressure while addressing multiple complex issues

⛳️ Requirements

• Associate or bachelor’s degree in nursing

• Over 3 years of clinical/acute care experience, including direct maternal patient care

• Current, unrestricted RN license in the state of residence

• Certified Case Manager (CCM) is preferred, or willingness and commitment to obtain within two years of hire

• Experience working with pregnant populations in clinical or home care settings

• Experience engaging with various age groups and diverse ethnic and socioeconomic backgrounds

• Proficiency in managing multiple computer systems and tools

• General understanding of the healthcare continuum

• Strong clinical foundation in maternal health, including pregnancy care and management of preeclampsia, gestational diabetes, and preterm labor

• Knowledge of postpartum recovery, covering both physical and mental health considerations

• Outstanding verbal and written communication skills with the ability to present effectively in group settings

• Capability to work efficiently towards departmental benchmarks

• Ability to thrive in a fast-paced, dynamic environment while managing multiple complex issues

• Must be legally authorized to work in the United States at the time of application
