This is a fully remote position, open to applicants in California.
• Deliver direct coaching support to clinical teams, both in-person and virtually.
• Assist care teams in acquiring and utilizing data to formulate clinical and operational action plans.
• Facilitate the adoption of technology solutions by health center teams to enhance practice transformation.
• Oversee multiple simultaneous clinical improvement initiatives.
• Identify, document, and disseminate best practice insights along with actionable recommendations.
• Bachelor's degree in healthcare or a related field; master's degree is highly preferred.
• At least three (3) years of experience supporting healthcare teams in a similar or relevant capacity is strongly preferred.
• Strong understanding of project lifecycle processes, with a high degree of independence and self-motivation.
• Proven experience in assisting individual clinicians, care teams, and clinic leaders with implementing practice changes within a clinical environment.
• Familiarity with change management and human-centered design; knowledge of advanced primary care concepts, chronic disease management, and experience with FQHC/safety net clinics is preferred.
• Exceptional written and verbal communication abilities.
