This is a fully remote position, open to applicants in California.
β’ Deliver direct coaching support to clinical teams, both in-person and virtually.
β’ Assist care teams in acquiring and utilizing data to formulate clinical and operational action plans.
β’ Facilitate the adoption of technology solutions by health center teams to enhance practice transformation.
β’ Oversee multiple simultaneous clinical improvement initiatives.
β’ Identify, document, and disseminate best practice insights along with actionable recommendations.
β’ Bachelor's degree in healthcare or a related field; master's degree is highly preferred.
β’ At least three (3) years of experience supporting healthcare teams in a similar or relevant capacity is strongly preferred.
β’ Strong understanding of project lifecycle processes, with a high degree of independence and self-motivation.
β’ Proven experience in assisting individual clinicians, care teams, and clinic leaders with implementing practice changes within a clinical environment.
β’ Familiarity with change management and human-centered design; knowledge of advanced primary care concepts, chronic disease management, and experience with FQHC/safety net clinics is preferred.
β’ Exceptional written and verbal communication abilities.
β’ Strong facilitation skills, including the capability to design and lead effective meetings.
