Post Care Transitions Specialist – Mid State

This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Facilitate care coordination, which includes treatment planning, assessment, monitoring, education, and referrals/linkages.

• Support individuals designated in the Transitions to Community Living Initiative Special Needs Populations.

• Focus on individuals with chronic, unresolved, or multifaceted physical, behavioral health, and social determinant challenges.

• Conduct care planning and discharge/transition planning from Adult Care Homes, inpatient units, and detention facilities.

• Collaborate with licensed Clinicians, Registered Nurses, and Peer Support Specialists on discharge, transition, and post-transition monitoring.

• Deliver care coordination/care management for assigned TCL individuals.

• Evaluate members in supportive housing for additional assistance and services required for sustained independent living.

• Perform assessments as necessary.

• Ensure timely and accurate documentation across various software platforms and State database systems.

⛳️ Requirements

• Must fulfill QP status: Fully licensed by the NC governing board overseeing Human Services professions; OR fully licensed RN authorized to practice in North Carolina by the NC Board of Nursing with four (4) years of full-time experience with the MH/DD/SA population; OR a college or university graduate with a Master’s degree in a human service field plus one year of full-time MH/DD/SA experience; OR a bachelor's degree in a human service field with two years of full-time MH/DD/SA experience; OR a bachelor's degree in a different field with four years of full-time MH/DD/SA experience.

• Alternatives for substance abuse professionals require supervised experience in alcoholism and drug abuse counseling as specified.

• A valid driver's license is mandatory.

• Must reside within Trillium’s Mid State Region: Anson, Guilford, Montgomery, Randolph, or Richmond counties.

• Ability to travel within the designated catchment area as required.

• Preferred: Experience and familiarity with peer support and working with individuals certified as peer support specialists.

• All candidates are required to pass a drug test as a condition of employment.
