Post-Acute Care Manager

This is a fully remote position, open to applicants in United States.

📋 Description

• Deliver care to patients transitioning to post-acute facilities

• Manage a caseload in line with productivity benchmarks

• Collaborate with patients, families or support networks, inpatient case management, and members of the interdisciplinary care team

• Facilitate the appropriate use of services, length of stay, and ensure safe discharge planning

• Generate daily transitions-of-care reports, assign tasks, and assess care goals

• Maintain weekly communication with skilled nursing facilities

• Assess the effectiveness of care plans with the Interdisciplinary Team

• Perform thorough assessments and complete necessary documentation

• Engage in patient outreach and record details in the electronic medical record

• Manage referrals and transitions among providers and care settings

• Offer education to patients and their families or caregivers

• Guide patients to the suitable next level of care and assist with discharge planning

• Lead monthly performance, quality, and readmission meetings with high-volume facilities

• Contribute to the development of standard operating procedures and policies

⛳️ Requirements

• Associate of Science in Nursing (ADN) (mandatory)

• Valid RN License in the state of employment or covered by compact (mandatory)

• Nursing compact/multistate license must be obtained prior to commencement

• Minimum of 2 years nursing experience (mandatory)

• At least 1 year of experience in skilled nursing environments (preferred)

• Bachelor of Science in Nursing (BSN) (preferred)

• Care management certification (preferred)
