Population Health Specialist III

This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Deliver clinical coordination in behavioral health for members facing intricate behavioral, social, and psychosocial challenges within population health programs.

• Evaluate the needs, risks, and obstacles of members concerning behavioral health and psychosocial elements.

• Organize services among various providers, care teams, and community resources.

• Utilize clinical judgment to facilitate engagement in suitable behavioral health and population health services.

• Track member progress and modify coordination strategies to enhance outcomes.

• Promote communication between multidisciplinary teams to guarantee coordinated care delivery.

⛳️ Requirements

• A Master's degree in health and human services or a related discipline.

• At least one (1) year of experience in a clinical environment working with the target population.

• Active North Carolina licensure as a Licensed Clinical Social Worker (LCSW), Licensed Clinical Mental Health Counselor (LCMHC), Licensed Marriage and Family Therapist (LMFT), Licensed Psychological Associate (LPA), Licensed Clinical Addictions Specialist (LCAS), or Registered Nurse (RN).

• A valid driver's license.

• Residency in North Carolina is required.

• Ability to travel within the catchment area as necessary.

• Successful completion of a drug test is required for employment.

• Preferred: five (5) years of post-Master's experience in a clinical setting with the BH/MH/SU/IDD population.

• Preferred: at least two (2) years of experience in Maternal health and Opioid treatment.
