Physician Owner - President, Affiliated Medical Group

This is a fully remote position, open to applicants in New York.

📋 Description

• Act as the sole or majority shareholder, director, and President of one or more professional corporations specific to the state.

• Provide clinical governance by endorsing clinical protocols, quality standards, and policies related to the scope of practice for NP-delivered care.

• Implement and uphold collaborative practice or supervision agreements with nurse practitioners as mandated by state legislation.

• Supervise credentialing, peer review, and clinical quality committees.

• Serve as or appoint a Medical Director for relevant programs.

• Approve clinical policies, standing orders, and care pathways.

• Maintain Medicare enrollment for the professional corporation and manage the reassignment of billing rights.

• Collaborate with the MSO on compliant operations while keeping exclusive authority over clinical decisions.

• Engage in periodic governance meetings, audits, and respond to payer or regulatory inquiries as necessary.

• Provide clinical oversight, ensure regulatory compliance, and offer professional supervision for an NP-led care model catering to skilled nursing and home health patients.

⛳️ Requirements

• MD or DO with an active, unrestricted medical license.

• Willingness to obtain licensure in additional target states.

• Active, unrestricted DEA registration.

• Board certification is preferred in Internal Medicine, Family Medicine, Geriatrics, or Physical Medicine & Rehabilitation.

• Current Medicare enrollment or eligibility without any revocations, opt-outs, or payment suspensions.

• Clean compliance profile: no OIG/SAM exclusions, no state board discipline, or significant malpractice history.

• Experience in supervising NPs and/or acting as Medical Director in post-acute, SNF, home health, telehealth, or value-based care environments.

• Comfort with friendly-PC arrangements, including stock transfer restrictions/succession agreements and MSO management structures.

• Previous ownership of a telehealth, RPM, or digital health company is a plus.

• Multi-state licensure or participation in IMLC is a plus.

• Familiarity with PDPM, RPM/CCM billing, and CMS Conditions of Participation is a plus.

• Experience with quality programs, UM/peer review, and payer credentialing is a plus.
