Physician Owner – President, Affiliated Medical Group

This is a fully remote position, open to applicants in Connecticut.

📋 Description

• Act as the sole or primary shareholder, director, and President of one or more state-specific professional corporations.

• Ensure clinical governance by authorizing clinical protocols, quality standards, and scope-of-practice policies for NP-led remote patient monitoring, chronic care coordination, and transitional care.

• Implement and sustain collaborative practice or supervision agreements with nurse practitioners as mandated by state regulations.

• Supervise credentialing, peer review, and clinical quality committees.

• Serve as or appoint the Medical Director for relevant programs.

• Approve clinical policies, standing orders, and care pathways.

• Maintain Medicare enrollment for the professional corporation and manage billing rights reassignment in accordance with program requirements.

• Collaborate with the MSO on compliant operations while retaining exclusive authority over clinical decisions under corporate practice of medicine regulations.

• Engage in periodic governance meetings, audits, and payer/regulatory inquiries as necessary.

• Provide governance and clinical leadership for physician-owned professional corporations that support an NP-led care model catering to skilled nursing and home health patients.

⛳️ Requirements

• MD or DO with an active, unrestricted medical license.

• Willingness to acquire licensure in additional target states.

• Active, unrestricted DEA registration.

• Board certification in Internal Medicine, Family Medicine, Geriatrics, or Physical Medicine & Rehabilitation is preferred.

• Current Medicare enrollment or eligibility without revocations, opt-outs, or payment suspensions.

• Clean compliance record: no OIG/SAM exclusions, no state board discipline, and no significant malpractice history.

• Experience in supervising NPs and/or fulfilling the role of Medical Director in post-acute, SNF, home health, telehealth, or value-based care environments.

• Comfort with friendly-PC arrangements, including stock transfer restrictions/succession agreements and MSO management structures.

• Prior ownership experience in a telehealth, RPM, or digital health company is a plus.

• Multi-state licensure or participation in IMLC is advantageous.

• Familiarity with PDPM, RPM/CCM billing codes 99453, 99454, 99457, and 99458, as well as CMS Conditions of Participation, is beneficial.

• Experience with quality programs, UM/peer review, and payer credentialing is a plus.
