Physician Owner – President, Affiliated Medical Group

This is a fully remote position, open to applicants in California.

📋 Description

• Act as the sole or primary shareholder, director, and President of one or more state-specific professional corporations.

• Ensure clinical governance by endorsing clinical protocols, quality standards, and scope-of-practice policies for care delivered by nurse practitioners.

• Implement and uphold collaborative practice or supervision agreements with nurse practitioners in accordance with state regulations.

• Supervise credentialing, peer review, and clinical quality committees.

• Serve as or appoint a Medical Director for relevant programs.

• Approve clinical policies, standing orders, and care pathways.

• Maintain Medicare enrollment for the professional corporation and manage reassignment of billing rights.

• Collaborate with the MSO to ensure compliant operations while retaining exclusive control over clinical decisions.

• Engage in periodic governance meetings, audits, and respond to payer or regulatory inquiries as necessary.

⛳️ Requirements

• MD or DO with an active, unrestricted medical license.

• Willingness to pursue licensure in additional target states.

• Active, unrestricted DEA registration.

• Board certification in Internal Medicine, Family Medicine, Geriatrics, or Physical Medicine & Rehabilitation is preferred.

• Current Medicare enrollment or eligibility without any revocations, opt-outs, or payment suspensions.

• Clean compliance record: no OIG/SAM exclusions, no state board disciplinary actions, and no significant malpractice history.

• Experience in supervising nurse practitioners and/or acting as a Medical Director in post-acute, skilled nursing facility (SNF), home health, telehealth, or value-based care environments.

• Comfort with friendly-PC arrangements, including stock transfer restrictions, succession agreements, and MSO management frameworks.

• Prior experience as a PC owner for a telehealth, remote patient monitoring (RPM), or digital health company is preferred.

• Multi-state licensure or participation in IMLC is preferred.

• Familiarity with PDPM, RPM/CCM billing (99453/99454/99457/99458), and CMS Conditions of Participation is preferred.

• Experience with quality programs, utilization management/peer review, and payer credentialing is preferred.
