Physical Therapist, Home Health

This is a fully remote position, open to applicants in Florida.

📋 Description

• Conduct initial and ongoing assessments for patients, including OASIS evaluation and documentation.

• Create and execute customized treatment plans in collaboration with physicians and clinicians.

• Modify treatment plans as necessary.

• Work alongside the interdisciplinary care team to enhance patient outcomes.

• Provide education and support to patients and their families/caregivers regarding therapy programs.

• Keep precise clinical records.

• Inform physicians of any changes in patient conditions.

• Oversee Physical Therapy Assistants in accordance with agency policies and state regulations.

• Engage in in-services, case conferences, and meetings.

• Recognize the need for additional services and make suitable referrals.

• Verify daily visit schedules.

• Share updates on patient progress and changes with physicians and care teams.

• Record all visits and progress notes on the same day of service.

⛳️ Requirements

• Valid state-specific PT license.

• Graduate from an accredited physical therapy program.

• Current BLS certification.

• At least one year of PT experience.

• Home health experience is preferred.

• Strong communication, organizational, and time management skills.

• Valid driver's license and access to a dependable, insured vehicle.

• Ability to visit homes that may not be accessible for wheelchairs.

• Adequate hearing, vision, and dexterity to perform physical assessments and demonstrate care techniques.

• Capability to walk, stand, stoop, and occasionally lift or reposition patients.

• Experience with electronic medical records is an advantage.
