Physical Therapist Assistant, Home Health

This is a fully remote position, open to applicants in Connecticut.

📋 Description

• Deliver physical therapy services in accordance with the defined Plan of Care.

• Operate under the guidance and oversight of a licensed Physical Therapist.

• Assist patients in enhancing their mobility, strength, balance, endurance, and self-sufficiency.

• Track patient progress and immediately notify the supervising Physical Therapist of any changes in condition.

• Review the Plan of Care prior to each appointment.

• Administer treatments as per physician directives and established clinical protocols.

• Instruct patients and caregivers on how to safely perform home exercise routines.

• Provide education on the proper use of walkers, canes, wheelchairs, prosthetics, orthotics, and other assistive devices.

• Reinforce patient objectives and motivate participation in therapeutic activities.

• Ensure accurate and prompt clinical documentation.

• Collaborate effectively with nurses, therapists, physicians, and other healthcare team members.


⛳️ Requirements

• Graduate from an accredited Physical Therapist Assistant program.

• Hold a current and unrestricted Connecticut Physical Therapist Assistant license.

• A minimum of six months of experience as a Physical Therapist Assistant is preferred.

• Excellent communication, organizational, and documentation skills.

• Capability to work independently within patients’ residences.

• Willingness to travel across Norwalk, Stamford, Greenwich, and the designated coverage area.

• Must have a valid driver's license and maintain an acceptable driving record, OR possess reliable transportation to commute between patient homes and assigned work locations.

• Must be able and willing to travel up to 50% of the time.

• Ability to frequently lift, push, pull, and support weights of up to 50 pounds, which includes positioning or transferring patients and relocating equipment.
