
Pharmacy Care Coordinator, MA – Contract to Perm

This is a fully remote position, open to applicants in United States.
• Conduct high-volume outbound calls to patients to introduce Evergreen's pharmacy service, building rapport and trust while guiding patients through the enrollment process.
• Provide clear and standardized messaging over the phone utilizing approved scripts and talking points to ensure consistent and informed enrollment decisions, while addressing patient questions and objections.
• Secure and document patient consent via phone and collect the necessary information for patient enrollment.
• Update patients' preferred pharmacy, initiate electronic prescriptions (eRxs) to the pharmacy partner, and accurately record consent or decline in the EHR.
• Manage outreach queues and follow-up schedules, ensuring multiple contact attempts per patient while prioritizing outreach based on urgency and objectives.
• Record call activities, outreach attempts, acceptances/declines, and coordination efforts in clinical and operational systems.
• Meet call center performance benchmarks, including call quality, productivity, average speed of answer, and enrollment/conversion goals.
• Maintain patient privacy and confidentiality in accordance with HIPAA regulations and organizational policies.
• Perform additional duties related to the role as assigned.
• Collaborate with patients from partner practices to engage them, obtain consent, and facilitate timely access to medications and pharmacist-led clinical services.
• Current Medical Assistant certification (CMA/RMA).
• Experience in a telephonic, call-center, or high-volume outbound patient/customer outreach environment.
• Excellent interpersonal and communication skills, with the ability to establish trust and engage patients over the phone.
• Background in healthcare, pharmacy, care coordination, or other patient support roles.
• Proficient and comfortable working within an electronic health record (EHR/EMR)—documenting encounters, reviewing medication lists, and making authorized updates.
• Strong organizational skills and attention to detail, capable of managing multiple patients, tasks, and follow-ups across an outreach queue.
• Knowledge of HIPAA regulations and a proven commitment to safeguarding patient privacy and PHI.
• Intermediate proficiency with MS Office Suite applications, including Outlook and Teams.
• Ability to work effectively in a primarily remote setting.
• Home internet connection must support a minimum download speed of 25 Mbps and upload speed of 10 Mbps.
