Pharmacy Care Coordinator – Clinical Pharmacy Operations

This is a fully remote position, open to applicants in Alabama, +3 more states.

📋 Description

• Assists in clinical outreach initiatives by making outbound calls and using portal chat to schedule appointments with the pharmacist team, addressing clinical gaps or medication concerns.

• Aids in cost of care initiatives by contacting providers and members to present lower-cost alternative medication options.

• Manages a designated caseload, which may involve follow-up with providers, pharmacies, and/or members to resolve medication recommendations, concerns, or inquiries.

• Independently supervises assigned cases and outreach efforts, ensuring timely follow-up, clear next steps, and comprehensive documentation.

• Aids members in navigating their Devoted Health benefits.

• Engages in sprints, pilots, and special projects as they are initiated.

• Collaborates with the wider pharmacy team to support common goals and coverage needs.

• Facilitates training and shadowing for new team members as they join.

• Takes part in team and pharmacy department activities and responsibilities.

⛳️ Requirements

• Minimum of 3 years of experience in healthcare or retail pharmacy.

• Active National PTCB Certification.

• Comfortable making a large volume of outbound calls to members, providers, and pharmacies.

• Able to manage daily responsibilities and tasks independently.

• Capable of frequently shifting between tasks, projects, and priorities.

• Highly organized and detail-oriented.

• Willingness to learn new software and adapt as tools and workflows change.

• Flexible and adaptable, eager to take on new or changing tasks with a positive attitude.

• A genuine team player who contributes fresh ideas, collaboratively works through solutions, and supports their teammates.

• Comfortable communicating with physicians, pharmacies, and patients, with a sincere passion for serving our members.

• Excels in a fast-paced and dynamic environment.

• Transparent communicator regarding work progress, successes, and challenges, with a keen focus on process improvement.
