Peer Support Specialist, Case Manager – Telehealth

This is a fully remote position, open to applicants in Oregon.

📋 Description

• Deliver peer support to members through both individual and group sessions, promoting community engagement, enhancing self-esteem, and aiding in long-term recovery.

• Assist members with case management to facilitate access to community resources and help them navigate obstacles to treatment.

• Conduct telephone outreach to potential members referred by partner organizations.

• Educate members on wellness and recovery techniques while acting as a mentor and role model.

• Ensure timely and accurate record-keeping in the EMR system.

• Collaborate closely with the clinical team to provide non-clinical support, personalized guidance, mentoring, and resources to members.

• A high school diploma or an equivalent qualification is mandatory.

• A valid Oregon OHA Peer Support Specialist certification is essential.

• Residence and employment within the state of Oregon is required.

• A private, HIPAA-compliant workspace equipped with reliable internet is necessary.

• Excellent interpersonal and communication abilities are crucial.

• Strong organizational skills and meticulous attention to detail, especially in documentation maintenance, are required.

• A minimum of 2 years of professional experience as a peer support specialist for individuals with substance use disorders is preferred.
