This is a fully remote position, open to applicants in Ohio.

📋 Description

• Execute care coordination and/or care management tasks aimed at enhancing members’ wellness and recovery.

• Recognize opportunities to involve members and their families in building a supportive recovery network.

• Create and deliver onsite education for psychiatric discharge planning at Recovery and Resiliency locations.

• Partner with Case Management Teams as an advocate for members in discharge planning education, addressing barriers, and facilitating service transitions.

• Serve as a resource for staff regarding decision-making and problem-solving.

• Initiate and sustain communication with assigned individuals and providers to assess members’ responses to services.

⛳️ Requirements

• AA/AS degree and at least 2 years of experience in the health services or behavioral health sector, or a peer support services role; or any combination of education and experience that provides an equivalent background.

• A valid and active certified recovery specialist/coach certification and/or peer/family support specialist certification is required.

• Strong preference for knowledge of care-coordination and case management principles.

• BA/BS or MBA is preferred.

• Candidates must live within a reasonable commuting distance from the job location(s), unless accommodations are provided as mandated by law.
