This is a fully remote position, open to applicants in Texas.
• Lead and coordinate the daily operations and workflow of the centralized Patient Services Coordinator team.
• Act as a supervisor, subject matter expert, and resource for resolving complex scheduling and patient coverage issues.
• Fulfill Patient Services Coordinator duties while assisting the larger team.
• Oversee staffing levels, productivity, scheduling efficiency, and key operational performance indicators.
• Analyze reports and dashboards to detect scheduling deficiencies, missed or incomplete visits, and coverage issues, and drive their resolution.
• Collaborate with Branch Directors, Clinical Managers, and field clinicians to enhance productivity, optimize scheduling, and ensure patient coverage.
• Track visit utilization and scheduling trends to minimize avoidable LUPAs and optimize clinical resource allocation.
• Balance workloads and staffing across designated agencies.
• Provide onboarding, training, coaching, and continuous development for Patient Services Coordinators.
• Conduct quality assessments of scheduling activities and documentation to ensure accuracy, consistency, and compliance.
• Streamline workflows, standardize procedures, and establish efficient scheduling practices.
• Facilitate communication between centralized scheduling, branch leadership, clinicians, patients, physicians, and the interdisciplinary care team.
• Relay operational trends, challenges, and opportunities to leadership while assisting in solution development.
• Support QAPI, performance improvement projects, and operational initiatives focused on scheduling and workforce optimization.
• Promote accountability, teamwork, urgency, and genuine care within the team.
• A minimum of 3 years of healthcare experience, ideally within home health, hospice, clinical operations, scheduling, or care coordination.
• Prior experience in a leadership, team lead, training, coaching, or mentoring role is preferred.
• Graduation from an accredited Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) program is preferred but not mandatory.
• Strong preference for candidates with previous home health experience and familiarity with clinical scheduling, visit utilization, and clinician productivity.
• Competence in using electronic medical records, scheduling systems, and reporting tools.
• Excellent leadership, coaching, relationship-building, organizational, analytical, problem-solving, written, and verbal communication skills.
• Current LPN/LVN license in the relevant state for clinical candidates.
• Current CPR certification for clinical candidates.
• Ability to uphold applicable professional and regulatory standards.
• If required to travel, a valid driver's license, reliable transportation, and automobile insurance compliant with agency requirements are necessary.
