Patient Referral Coordinator

This is a fully remote position, open to applicants in Pennsylvania, +1 more state.

📋 Description

• Arrange specialist appointments for patients who have been referred and verify the details with both the patient and specialty office.

• Send appointment reminders through phone calls, text messages, and various other channels to minimize missed appointments.

• Address no-shows and cancellations by reaching out to patients and rescheduling as necessary.

• Maintain and manage waitlists to fill openings as they arise.

• Schedule follow-up appointments and ensure patients complete their recommended care.

• Inform patients about the advantages of utilizing the FQHC’s onsite pharmacy, which includes care coordination, convenience, and enhanced access to medications.

• Collaborate with specialty care offices to guarantee that referral documentation, records, and results are shared accurately and punctually.

• Keep referral and scheduling records up-to-date and precise within the patient management system.

• Manage patient information in compliance with HIPAA and NuvemRx policies.

• Work together with health center teams, specialty offices, and pharmacy staff to effectively coordinate patient care.

⛳️ Requirements

• A minimum of 2 years of experience in a healthcare, medical office, pharmacy, or patient services environment; experience with scheduling, referral coordination, or intake is preferred.

• Excellent verbal and written communication skills, exhibiting a patient and service-oriented approach.

• Proficiency in EHR, scheduling, or referral management software, along with general computer skills.

• Capability to handle a large volume of appointments and referrals while ensuring accurate and organized records.

• High school diploma or equivalent required.

• Experience in an FQHC, community health, or care coordination setting is preferred.

• Understanding of specialty care referral workflows is preferred.

• Bilingual proficiency in English and Spanish is preferred.
