This is a fully remote position, open to applicants in United States.
• Assist patients in navigating the healthcare system, which includes scheduling appointments, making referrals, and coordinating follow-up care.
• Act as the primary contact for patients, responding to inquiries and concerns with professionalism and empathy.
• Help patients comprehend treatment plans, discharge instructions, and available healthcare resources.
• Work collaboratively with clinical staff, physicians, and administrative teams to ensure seamless continuity of care.
• Keep patient records accurate and current within the electronic health record (EHR) system.
• Identify and assist in overcoming obstacles to care, such as insurance issues, transportation challenges, and social determinants of health.
• Track patient progress and proactively follow up to ensure compliance with care plans.
• A high school diploma or GED is required.
• An associate's or bachelor's degree in healthcare, social work, or a related field is preferred.
• Previous experience in a healthcare, patient services, or care coordination role is strongly preferred.
• Knowledge of medical terminology, EHR systems, and healthcare workflows is essential.
• Excellent interpersonal and communication skills with a focus on patient care are necessary.
• Capability to multitask, prioritize, and resolve problems in a fast-paced environment.
• Understanding of community health resources, insurance processes, and care management principles is important.
