Patient Enrollment Specialist

This is a fully remote position, open to applicants in United States.

📋 Description

• Make a significant number of outbound calls to patients referred by primary care providers or health plans.

• Inform patients about Diverge Health’s care management programs.

• Address patient inquiries and resolve objections regarding program participation.

• Establish trust with patients and customize conversations to meet their individual needs.

• Arrange follow-up appointments and facilitate patient onboarding with Health Coaches and internal teams.

• Guarantee a smooth transition for patients joining Diverge Health’s programs.

• Keep precise and organized records of patient interactions using CRM systems such as Salesforce.

• Monitor outreach metrics and assist in achieving team performance objectives.

• Report directly to the Director of Central Care Operations.

• Connect patients with resources for managing chronic conditions such as diabetes, hypertension, COPD, CHF, asthma, and mild to moderate depression and anxiety.

⛳️ Requirements

• 1-2 years of experience in high-volume cold outreach via telephone.

• Preferred experience in the healthcare sector.

• Outstanding communication and interpersonal skills paired with a high degree of empathy.

• Familiarity with CRM systems (e.g., Salesforce) and call management tools, including basic administrative functions.

• General understanding of healthcare system operations, encompassing payers, Medicaid vs. Medicare, and fee-for-service (FFS).

• Experience in chronic disease management or knowledge of healthcare best practices for common chronic illnesses is advantageous.

• Strong organizational abilities and capacity to manage multiple tasks concurrently.

• Must have authorization to work in the United States without employer sponsorship, both now and in the future.

• Must be able to work within Eastern and Central time zone operations; hiring is not available in Alaska and Hawaii.
