Patient Engagement Specialist

This is a fully remote position, open to applicants in Arizona, +9 more states.

📋 Description

• Interact with potential clients and document all engagements in the Client Relationship Management (CRM) system.

• Conduct 60–100 outbound calls each day to reach prospective clients.

• Inform potential clients about Enhanced Care Management (ECM) services.

• Support enrollment by evaluating client needs and connecting them with the care management team.

• Communicate vital information and client preferences to the care team to ensure personalized care planning.

• Aid clients in completing psychosocial assessments and crafting care plans with actionable objectives.

• Examine medical records to document client needs and suggest suitable interventions.

• Utilize Motivational Interviewing, Harm Reduction, and Trauma-Informed Care principles.

• Attend weekly Team Operations Meetings to coordinate and enhance care delivery.

⛳️ Requirements

• Must be available to work Monday through Friday from 8:30 am to 5:00 pm Pacific Time, with some flexibility for evenings and weekends.

• Current California Licensed Vocational Nurse (LVN) license, or Medical Assistant certificate from an accredited organization, or Bachelor’s degree in a relevant field.

• At least 2 years of experience in telephonic outreach or enrollment in a high-call volume setting.

• Experience working with complex care clients and ECM Populations of Focus.

• Strong oral and written communication abilities.

• Positive interpersonal skills to engage effectively with clients.

• Proficient with technology, including video conferencing tools, Google Workspace, Slack, and electronic health records.

• Previous experience in a remote work environment.

• A quiet and secure workspace to maintain member privacy.

• Capability to test internet speed to ensure smooth application performance.

• Bilingual (English/Spanish) is highly preferred.

• Applicants must reside in Arizona, California, Colorado, Florida, Georgia, Illinois, Nevada, North Carolina, Oregon, Texas, or Washington.

• A background check is required before employment, which may include a drug test or other health screenings.
