This is a fully remote position, open to applicants in Texas.
• Conduct comprehensive research on assigned patients to assess healthcare needs and usage, identify enrollment obstacles, and discover methods for engaging caregivers.
• Execute telephonic outreach to caregivers for enrolling children and ensuring a smooth onboarding process.
• Carry out telephonic communication with healthcare professionals, including DME companies, home health agencies, pharmacies, and pediatricians.
• Utilize relationships with healthcare professionals to assist in re-engaging enrolled patients.
• Manage incoming calls from caregivers, health plan service coordinators, and hospital discharge planners throughout the enrollment process.
• Acquire knowledge of the Supplemental Security Income (SSI) program and reach out to potentially eligible caregivers.
• Collaborate with Provider Engagement Specialists on targeted outreach initiatives at PCP offices, residential facilities, and other relevant locations.
• Work in partnership with the engagement pod, sharing responsibility for enrollment and onboarding.
• A minimum of 2 years of experience in healthcare, specifically in care coordination, community health, patient engagement, customer service, or sales.
• Demonstrated success in a centralized contact center setting.
• Strong documentation skills; prior experience with a CRM system is advantageous.
• Exceptional collaboration skills.
• Excellent active listening and communication abilities.
• Capability to customize value propositions for diverse audiences and navigate complex objections.
• Proficient in building trust and establishing relationships with caregivers and healthcare organizations.
• Enthusiasm and skill for research and data analysis.
• Strong critical thinking and innovative problem-solving capabilities.
• Ability to work autonomously at a high level.
• Access to reliable transportation and a willingness to travel to the field as needed.
• Reliable internet access is mandatory.
• Bilingual in Spanish is required.
