Patient Care Coordinator

This is a fully remote position, open to applicants in United States.

📋 Description

• Oversee a designated panel of patients participating in CCM and RPM programs

• Conduct regular outbound calls and follow-ups with patients

• Foster trusting relationships and motivate engagement in care plans

• Offer fundamental health coaching, education, and guidance in accordance with clinical protocols

• Recognize patient issues, obstacles to care, or changes in health status and escalate as needed

• Assist patients with inquiries regarding care management and remote monitoring

• Analyze and monitor RPM data, alerts, readings, and trends

• Adhere to workflows for readings that necessitate attention or escalation

• Help patients with RPM devices and related technology

• Follow up on missing readings, device problems, and abnormal results

• Accurately document monitoring activities and patient interactions

• Aid in the creation, execution, and updates of personalized care plans

• Track patient objectives, follow-ups, appointments, medications, and care requirements

• Facilitate communication among patients, providers, caregivers, and members of the care team

• Identify care gaps and ensure appropriate follow-up actions

• Coordinate referrals and link patients with healthcare resources as directed

• Maintain precise records within healthcare platforms

• Safeguard patient information and uphold HIPAA compliance

• Adhere to clinical, operational, and documentation protocols

⛳️ Requirements

• Prior experience in a healthcare, patient care, medical assistant, nursing, care coordination, or similar setting

• Strong comprehension of patient communication and healthcare workflows

• Proficiency in both Russian and English

• Exceptional verbal and written communication abilities in English and Russian

• High levels of empathy, patience, and the capability to establish rapport with patients

• Outstanding organizational skills and follow-through

• Ability to work independently in a remote setting

• Strong computer proficiency and capacity to learn various healthcare technology platforms

• Reliable internet connection and a professional remote workspace

• Dedication to patient privacy and adherence to HIPAA compliance

• Preferred: RN, LPN/LVN, CMA, CCMA, or equivalent healthcare certification

• Preferred: Experience with Chronic Care Management (CCM)

• Preferred: Experience in Remote Patient Monitoring (RPM) or telehealth

• Preferred: Experience managing a patient panel or caseload

• Preferred: Experience with chronic conditions such as diabetes, hypertension, cardiovascular disease, respiratory conditions, or other long-term health issues

• Preferred: Familiarity with EMR/EHR systems and remote care platforms

• Preferred: Experience with RPM devices and patient monitoring technology

• Preferred: Prior patient education or health coaching experience

• Preferred: Experience in coordinating referrals, appointments, medications, or follow-up care
