This is a fully remote position, open to applicants in Michigan.

📋 Description

• Perform virtual wellness visits for adults and/or children, health assessments, and associated healthcare services.

• Review patient caseload and prepare the necessary technology for service delivery.

• Utilize technology to manage the daily schedule efficiently.

• Collaborate with clinical in-home team members during patient home visits.

• Establish welcoming relationships with a diverse patient population.

• Gather pertinent patient information and provide necessary services.

• Execute health risk assessments, medication reviews, and cognitive screenings.

• Educate patients regarding their health and wellness.

• Work alongside Sprinters to assess vital signs and organize blood draws.

• Address specific patient needs through dedicated tasks.

• Facilitate care coordination and management, including preventive care measures, medication oversight, specialist referrals, community resource connections, and documentation of findings.

⛳️ Requirements

• Board Certified as a Family Nurse Practitioner.

• Active Family Nurse Practitioner license in Louisiana.

• Previous experience in conducting wellness visits.

• Demonstrates professionalism, integrity, accountability, confidentiality, care, and compassion.

• Flexibility to engage in last-minute problem solving and innovative thinking.

• Proficient with technology and comfortable using laptops or mobile devices for documentation.

• Familiarity with HIPAA secure messaging applications for care coordination.

• Excellent written and verbal communication skills.

• Capable of working autonomously or as part of a team.

• Must successfully pass a national background check and valid clinical license verification.

• Completion of required application inquiries regarding state licensure, wellness visit experience, Medicaid enrollment, virtual Family Nurse Practitioner experience, notice period, prescriptive authority, and board certification.
