Part Time Virtual Family Practice Nurse Practitioner, OH Licensed

This is a fully remote position, open to applicants in Ohio.

📋 Description

• Review the daily caseload and prepare technology and tools for patient care.

• Navigate the daily schedule utilizing modern technology.

• Collaborate with clinical in-home team members (Sprinters) who visit patients’ homes.

• Establish welcoming connections with a diverse range of patients.

• Gather pertinent patient information and perform necessary services.

• Conduct virtual wellness visits for adults and/or pediatrics.

• Carry out health risk assessments, medication reviews, and cognitive screenings.

• Provide patients with educational resources regarding health and well-being.

• Assess vital signs in collaboration with Sprinters.

• Organize blood draws and address specific patient needs.

• Offer care coordination and management services.

• Implement preventive care interventions and manage medications.

• Coordinate specialist referrals and connect patients with community resources.

• Document clinical findings accurately.

⛳️ Requirements

• Board Certified as a Family Nurse Practitioner.

• Active Family Nurse Practitioner license in Louisiana.

• Previous experience in conducting wellness visits.

• Demonstrated professionalism, integrity, accountability, confidentiality, care, and compassion.

• Openness to last-minute problem solving and innovative thinking.

• Proficient in using laptops or mobile devices for charting and HIPAA-secure messaging apps for care coordination.

• Strong written and verbal communication abilities.

• Capability to work independently or collaboratively within a team environment.

• Successfully pass a national background check and valid clinical license verification.

• Must be eligible to work remotely within the United States.
