Part-Time Home Care & Hospice Virtual Care Coordinator, RN

This is a fully remote position, open to applicants in Ohio.

📋 Description

• Perform virtual evaluations and oversee patients' conditions from a distance.

• Create and manage personalized care plans in collaboration with the interdisciplinary team.

• Offer virtual education and assistance to patients, families, and caregivers about symptom management and end-of-life care.

• Coordinate with field nurses, physicians, social workers, and other team members.

• Quickly address urgent virtual consultations and assess patient needs.

• Utilize telehealth technology for regular check-ins and follow-ups with patients and their families.

⛳️ Requirements

• Over 5 years of experience as an RN in Home Care, Hospice, ER, and/or Triage.

• Current RN license in a Compact State (eNLC).

• BSN degree from an accredited nursing education program.

• Experience with Epic or Home Care EMRs is essential.

• Excellent communication and interpersonal skills to foster rapport with patients and families virtually.

• Capability to work independently as well as part of a collaborative remote team.

• Flexibility to adapt to swift changes to meet business or department requirements.

• Prior experience in Patient/Customer Service.

• Availability for a Saturday and Sunday schedule from 2:00 p.m. to 12:30 a.m. EST.
