Part-Time Home Care & Hospice Virtual Care Coordinator, RN

This is a fully remote position, open to applicants in Florida.

📋 Description

• Provide telehealth hospice services and offer tailored support to patients and their families.

• Perform virtual evaluations and monitor the health status of patients remotely.

• Create and coordinate personalized care plans in collaboration with the interdisciplinary team.

• Offer virtual education and assistance on symptom management and end-of-life care.

• Collaborate with field nurses, physicians, social workers, and other team members.

• Address urgent virtual consultations and prioritize patient needs.

• Utilize telehealth technology for regular check-ins and follow-up interactions with patients and their families.

⛳️ Requirements

• Minimum of 5 years’ experience in Home Care, Hospice, ER, and/or Triage RN roles.

• Current RN license in a Compact State (eNLC).

• A BSN obtained from an accredited Nursing education program is mandatory.

• Experience with Epic or Home Care EMR is essential.

• Excellent communication and interpersonal skills to foster rapport with patients and families in a virtual setting.

• Capability to work autonomously and as part of a collaborative remote team.

• Flexibility to adapt to swift changes to fulfill business and departmental requirements.

• Experience in Patient/Customer Service is preferred.
