This is a fully remote position, open to applicants in California.

📋 Description

• Deliver advanced clinical assistance to the Parenteral Services team and act as a clinical resource for nurses and multidisciplinary staff.

• Aid nursing care plans, clinical decision-making, complex patient management, vascular access, infusion complications, and laboratory monitoring.

• Support hospital-to-home patient transitions and engage in multidisciplinary case reviews.

• Educate nurses and assist with urgent clinical inquiries during business hours, as well as provide after-hours support as assigned.

• Collaborate with Staffing Coordinators, Laboratory Coordinators, Pharmacists, Dietitians, and the RN Supervisor.

• Assist with high-risk Start of Care planning, workflow oversight, clinical prioritization, and readiness for weekends and holidays.

• Facilitate care coordination for patients facing clinical complications.

• Encourage adherence to company policies and regulatory requirements.

• Take part in quality improvement projects, incident evaluations, corrective measures, and the execution of clinical protocols.

• Examine documentation for clinical completeness and consistency, while identifying opportunities for workflow enhancements.

• Strengthen patient education related to TPN administration, line care, medication storage, infection prevention, and troubleshooting of infusion equipment.

• Address patient concerns and maintain continuity of care during therapy adjustments.

• Execute additional departmental responsibilities as assigned and promote accountability, integrity, collaboration, and excellence in service.


⛳️ Requirements

• In-depth knowledge of TPN preparation, administration, equipment usage, management of line complications, and infection control practices.

• Proficiency in TPN laboratory monitoring standards and clinical parameters.

• Skilled in using, managing, and troubleshooting Central Lines, PICC lines, and implanted ports.

• Strong leadership and mentorship capabilities.

• Exceptional critical thinking and problem-solving skills.

• Effective verbal and written communication abilities.

• Proficiency in electronic health records (EHR) systems and other digital tools.

• Valid driver’s license with the capacity to travel within the designated territory.

• Certification in Nutrition Support (CNSC) is highly preferred.

• Experience in home infusion nursing and patient education is advantageous.

• Familiarity with AmeriPharma Pharmacy Specialty Infusion Services guidelines is a plus.

• Ability to regularly lift and/or move up to 20 pounds and occasionally lift/move up to 30 pounds.
