Palliative Nurse, RPN

This is a fully remote position, open to applicants in Canada.

📋 Description

• Perform scheduled visits within the community to provide care for patients across various environments.

• Attend to patients of all ages, accommodating a wide range of care complexities.

• Deliver ethical and safe nursing care in accordance with the scope of practice.

• Evaluate patients’ specific needs utilizing suitable assessment tools and methodologies.

• Implement, consistently monitor, and modify care plans as necessary.

• Establish rapport and trust with patients through a collaborative approach.

• Maintain and regularly update professional patient records pertaining to encounters and care plans.

• Provide necessary on-call support, including after hours.

⛳️ Requirements

• Registration as a Registered Practical Nurse with the CNO in good standing.

• Mandatory experience in palliative care.

• Required post-graduate education in palliative care.

• A valid driver’s license and access to a vehicle are essential.

• Experience in community care delivery is advantageous.

• Dependability and adaptability are crucial.

• Willingness to travel to and from patient homes within a specified area.

• Experience working with vulnerable populations.

• A strong passion for making a positive impact in the community.

• Required availability for on-call support, including after hours, to ensure patient safety and address care needs.
