This is a fully remote position, open to applicants in New Jersey.
• Oversee planned telephone and/or virtual check-ins with patients under the guidance of the supervising RN/provider.
• Perform thorough reviews of high-acuity patient charts to evaluate the clinical suitability for hospice care.
• Track and record symptoms reported by patients, changes in their condition, medication issues, and care requirements.
• Educate patients and caregivers on medications, symptom control, disease processes, and available support services.
• Report any changes in patient condition or urgent issues to the relevant RN, provider, or interdisciplinary team member.
• Facilitate care coordination for patients among caregivers, physicians, nurses, pharmacies, home health agencies, hospice/palliative care providers, and other healthcare professionals.
• Adhere to established clinical protocols, care plans, and organizational policies.
• Offer emotional support and empathetic communication to patients and families facing serious illness.
• Aid in medication reconciliation and identify potential medication-related issues for review by the appropriate clinician.
• Ensure accurate, timely, and comprehensive documentation in the electronic medical record (EMR).
• Engage in interdisciplinary team meetings and communicate pertinent patient updates.
• Assist with referrals, follow-up appointments, supplies, and community resources as needed.
• Safeguard patient privacy and remain compliant with HIPAA and relevant federal and state regulations.
• Execute other tasks within the LPN/LVN scope of practice as assigned.
• Valid, unrestricted LPN license in the relevant state(s).
• Graduate from an accredited practical nursing program.
• Preferred prior experience in palliative care, hospice, home health, oncology, geriatrics, chronic disease management, or a related field.
• Excellent clinical assessment, communication, organization, and documentation abilities.
• Comfortable providing patient support through telephone and/or virtual platforms.
• Capability to identify changes in patient condition and escalate concerns effectively.
• Proficient in electronic medical records and standard computer applications.
• Strong interpersonal skills with the ability to communicate with patients and families with empathy and professionalism.
• Ability to work independently while effectively collaborating with an interdisciplinary healthcare team.
