This is a fully remote position, open to applicants in United States.
• Offer Social Work assistance to the Palliative Care Program
• Conduct telephonic and video consultations with patients and their caregivers
• Build trust and foster strong relationships with patients
• Collaborate with clinicians, nurses, care coordinators, and interdisciplinary team members
• Perform Social Work interventions and psychosocial evaluations
• Develop customized care plans tailored to the needs of members
• Provide emotional support and assistance to caregivers
• Address complex family dynamics and support issues
• Facilitate goals of care discussions and advanced care planning
• Engage in long-term care planning and placements
• Initiate and monitor behavioral health referrals
• Investigate any concerns related to patient abuse or neglect
• Assist patients, caregivers, and families in achieving their goals of care and addressing social factors in advanced care planning
• Organize and participate in family meetings and joint visits
• Take part in team huddles, panel reviews, interdisciplinary team rounds, and program development initiatives
• Cultivate relationships and coordinate efforts with local market and internal stakeholders
• Record all clinical interactions in the electronic health record
• Master’s degree in social work and a valid active license
• At least 2 years of experience in palliative or hospice care, with expertise in leading goals of care discussions, assisting in advanced care planning, and providing support through grief, loss, and bereavement
• Certification in hospice and palliative social work, or a commitment to obtain it within one year of employment
• Proficient with technology and adaptable to a fast-paced work environment
• Self-motivated and capable of thriving in a remote work setting
• Excellent interpersonal and communication abilities
• Comfortable interacting with patients via video and phone
• Bilingual in English and Creole, Spanish, or Vietnamese (preferred)
• Background in health insurance (Medicare Advantage experience is a plus) (preferred)
• Experience in geriatrics (preferred)
