Outpatient LPN

This is a fully remote position, open to applicants in Ohio.

📋 Description

• Provide nursing care to patients and their families throughout the health illness continuum in various healthcare environments.

• Assist the centralized Medication Refill Call Center as part of the Patient Advice and Care Team.

• Conduct initial, ongoing, and functional health status evaluations.

• Gather, assess, and record both subjective and objective patient information.

• Recognize and convey assessment results to the relevant healthcare team members.

• Create, implement, coordinate, and document personalized care plans and nursing interventions.

• Deliver direct patient care and guide patients and families through the healthcare system.

• Assess and document patient responses, updating care plans as necessary.

• Assign nursing tasks in accordance with established criteria and state regulations, reviewing delegated work.

• Ensure continuity of care and engage in the professional development of peers and other staff members.

• Mentor employees and offer constructive feedback.

• Take part in process improvement initiatives, promoting clinical quality, customer satisfaction, and financial performance.

• Serve as a patient advocate and collaborate with healthcare providers.

• Oversee personnel, supplies, and equipment to uphold a safe and efficient working environment.

• Communicate instructions, handle minor scheduling, inspect work, and act as a resource for associates.

⛳️ Requirements

• High School diploma or GED (Required).

• BLS - Basic Life Support certification from the American Heart Association.

• LPN - Licensed Practical Nurse certification from the National Council of State Boards of Nursing.

• Preferred 3–5 years of experience as an LPN.

• Ideally, at least 1 year of experience in Primary Care.

• Excellent communication and critical thinking abilities.

• Comfortable utilizing technology in a dynamic, team-oriented environment.

• Must be able to work from Ohio.
