This is a fully remote position, open to applicants in United States.
• Evaluate customer needs, create personalized care plans, and document interventions and results in Cigna medical management systems.
• Work in partnership with customers and healthcare providers to set goals, implement interventions, and develop evidence-based care plans.
• Utilize motivational interviewing, behavior modification techniques, and collaborative decision-making to enhance engagement and health outcomes.
• Empower customers to effectively navigate the healthcare system and engage with their providers.
• Facilitate care coordination among customers, caregivers, providers, and internal teams through phone and digital communication channels such as email, text, and chat.
• Collaborate with multidisciplinary teams, including pharmacists, nutritionists, behavioral health clinicians, Medical Directors, and Medical Management programs.
• Monitor, analyze, and document daily activities, volume, and outcomes to support performance and quality objectives.
• Adhere to established policies, procedures, and Case Management performance standards.
• Navigate various systems and applications in a dynamic, customer-oriented environment.
• Complete necessary training and maintain clinical expertise, licensure, and certifications.
• Current, unrestricted Registered Nurse (RN) license in your state of residence.
• At least two (2) years of full-time direct patient care experience as an RN.
• Preferred specialty case management experience.
• Oncology experience preferred for specialized roles.
• Capability to obtain a URAC-recognized Case Management certification within four (4) years of hire.
• Strong clinical judgment with the ability to assess risk, prioritize care, and respond decisively.
• Excellent verbal and written communication abilities across phone and digital platforms.
• Proficient in Microsoft Word, Excel, Outlook, and online research tools.
• Ability to manage conflict, collaborate across teams, and engage with diverse populations.
• Strong organizational skills, adaptability, and comfort in managing multiple complex cases simultaneously.
• Ability to maintain clinical expertise, licensure, and certifications.
• Must reside within the United States.
• Home internet connection through cable broadband or fiber optic service with a minimum speed of 10Mbps download/5Mbps upload when working remotely.
