Nurse Practitioner – Weekend On-Call

This is a fully remote position, open to applicants in United States.

📋 Description

• Evaluate the medical and mental health requirements of patients, creating care plans that proactively address conditions and obstacles to care.

• Ensure adherence to regulations set by local, state, and federal agencies pertaining to clinical services.

• Prescribe medications, and arrange for lab tests, diagnostic procedures, and consultations.

• Monitor patients’ adherence to treatment and their response, making necessary adjustments to treatment plans.

• Collaborate with RN case managers as part of a cohesive care team.

• Deliver tailored, coordinated healthcare for long-term care residents enrolled in specialized Medicare Advantage programs.

• Manage the treatment of patients with both acute and chronic conditions, exercising independent judgment while consulting with a supervising physician when necessary.

• Partner with facility associates and health plan leadership to implement the national care model.

• May offer training and guidance to facility staff.

⛳️ Requirements

• A valid FNP, AGNP, AHACNP, or PA license is required.

• A degree from an accredited APRN or PA program is mandatory.

• A minimum of 3 years’ experience in clinical nursing or rehabilitation in geriatric populations is necessary.

• Experience with Electronic Health Records is essential.

• Proficient understanding of Microsoft applications, including Word, Outlook, and Excel.

• A Master’s degree or higher is preferred.

• Prior experience in primary care within geriatric medicine is advantageous.

• A minimum of 3 years’ experience in long-term care is preferred.

• An APRN Advanced Practice RN certification/license is required.
